Download the Consumer Complaint Form to lodge your complaint with us via fax or post or to lodge your dispute online visit our complaint wizard.

If your complaint involves more the 2 complainants you may use the Disclosure Statement to add more complainants.

If you have a person acting on your behalf you can notify us by completing the Third Party Authority form.

If you would like to add an additional party to the complaint, but they are not part of the complaint, please use the Authority for Additional Party form.
